Life Always Matters |
HOSPICE IS:
|
A concept of end-of-life care, not a place. Emphasis is on quality of
life during the living time between a terminal diagnosis and death. Hospice
accepts death as a natural part of life, seeking neither to hasten nor
prolong the dying process. This carefully developed concept provides services
for the physical, emotional, and spiritual needs of the whole person and
their families. A team
of professionals and trained volunteers provide
care mainly in the home. This enables the patient to retain the dignity
of controlling their lives, in comfort, surrounded by loved ones, during
this time |

YOUR HOSPICE CARE TEAM:

A PERSON WE SERVE:
|
- Is a McDowell County resident
- Wishes to remain at home (including living at assisted living facilities
and nursing homes)
- Their Personal Physician and the Hospice Medical Director certifies
appropriateness for hospice care (Admission Standards & Diagnosis)
- Patient chooses to receive hospice care rather than curative treatments

SERVICES PROVIDED BY HOSPICE:

- Individualized Plan of Care
- Accessible care team—24 hours a day, 7 days a week
- Professional nursing visits (Hospice Services)
- Personal care assistance
- Emotional support
- Medications, medical supplies, and equipment related to the admitting
diagnosis
- Teaching family how to care for the patient
- Periods of relief for caregivers by volunteers
- Spiritual support
- Physical therapy when needed
- Linking to community resources
- Short term inpatient care if pain or symptoms cannot be managed
at home, or the caregiver needs respite time
- Bereavement support for surviving loved ones
- Grief Support Groups
- “Show Me Thy Faith” partnerships with local churches

DID YOU KNOW?

- Hospice services are paid for by Medicare, Medicaid, health insurance
plans, memorial gifts, contributions, bequests, and fundraising events
- Hospice Services may be requested by the patient, a family member,
or a friend (with prior permission of patient and family)
- Hospice is not just for cancer patients—any
life-limiting Illness such as heart, lung, or kidney disease is included
- The earlier that hospice is called, the greater the benefit that can

- Hospice can provide education on a variety of topics related to end-of-life
care
- A lending library of videos, books, and pamphlets on dying and grief
is provided for anyone in the community—topic areas include:
- Adult Issues
- Suicide
- Children
- Adolescents

What You Can Do

VOLUNTEER |
For some of the most rewarding time you could spend in your
community, think about volunteering for hospice. In-depth training is
provided and opportunities to
volunteer are varied:
- Patient/Family-
to provide patient companionship and caregiver support
- Office- to
provide clerical support
- Bereavement- to maintain contact and support patient families
Committee- to serve in various roles
- Service- to do repairs or make afghans, pillows, cakes, etc. for
patients

Donations are a lifeline of support for hospice services,
especially when a portion of hospice services may not be reimbursed. When
you make a contribution through any of the ways listed below,
you have supported a family in need. You may donate through:
