PRIMARY FUNCTION:   The Medical Social Worker works under the general direction of the Family Resource Coordinator and in coordination with the Interdisciplinary Team in the provision of supportive services to help meet patient/family social, emotional and economic needs in relation to the impact of a terminal illness and the grief and bereavement issues that follow.  The Medical Social Worker provides social services and bereavement activity as it relates to terminally ill patients and their families.  Social services and bereavement activities include treatment planning, consultation with other staff members, and searching for resources to assist the patient and/or family.

SUPERVISOR:  Family Resource Coordinator

QUALIFICATIONS:  
--   Master’s or Bachelor's degree in Social Work from a school accredited by the Council of Social Work Education;

--   At least one year Experience in a health care setting;
--   Ability to work with and manage a wide array of resources;
--   Demonstrates effective communication skills, and a willingness to effectively work with members of the Hospice Interdisciplinary Team and volunteer and office staff;
--   Clinical skills related to grief and loss;
--   Knowledge of the dynamics of grief and bereavement;
--   Two years of clinical experience preferred;
--   North Carolina Driver’s License;
--   Available vehicle for transportation.

DUTIES AND RESPONSIBILITIES:**
--   Identify and meet the needs of patients, families, and volunteers through listening, problem-solving, teamwork, communication, and other various methods;
--   Demonstrate compassionate, quality care, education and professionalism with the patients and families;
--   Demonstrates knowledge and familiarity with regulatory standards (including Medicare, DFS, etc.);

--   Effectively coordinate and communicate with team members to develop goals and implement interventions to the patient and bereavement plans of care (including level of care changes, transfers, etc.);
--   Provide necessary counseling, social and/or bereavement services to the patient/family experiencing emotional, social or economic conflict;
--   Effectively assess patient/family social and emotional factors in order to estimate the patient and family’s capacity and potential to cope with the terminal illness and death;
--   As a member of the Interdisciplinary Team, assist team members in understanding the significant social, emotional, and grief factors/issues related to care of the terminally ill patient and the family;
--   Effectively assist patient, family and staff in utilizing community resources to meet patient/family needs;
--   Effectively assess patient/family ability to meet financial obligations and offer appropriate resources and activate as necessary;
--   Evaluate patient’s insurance coverage and options in regard to the terminal illness and hospice care;
--   Assist in discharge planning from hospice service when care is no longer appropriate;
--   Attend patient team meetings and facilitate discussion of issues related to the psychosocial and bereavement component of hospice care;
--   Ensure that documentation of services meets regulatory and organizational standards and is handled in a timely manner;
--   Provide effective bereavement follow-up services as defined by Hospice of McDowell County procedures;

--   Participate in orientation, continuing education and in-service training programs for professional staff;
--   Participate in the orientation, training and coordination of volunteers working in the Hospice programs as appropriate;
--   Must be available and accessible to work outside the normal workday of 8:00 AM to 5:00 PM, including covering patient and family emergencies as they arise;
--   Participate in agency and community programs as requested to promote professional growth and understanding of hospice care;
--   Maintain library information;
--   Maintain statistical records and prepare reports in a timely and professional manner
--   Maintain confidentiality (including meeting HIPAA requirements) of all information relating to Hospice patients/families;
--   Perform other tasks as assigned or requested by the Family Resource Coordinator.

Primary Registered Nurse 1st Shift/32 hours 8 to 5 weekdays/rotating call

The position available in April/May 2008 will be a Primary RN position with a caseload and call responsibilities, however a portion, 10 to 15 hours per week, will be working on referrals, admissions, and medical community liason.

Position Summary: Responsibilities include caseload assignment; interaction with patient and families on a scheduled basis at their place of residence to manage and educate regarding disease progression; symptom management; signs and symptoms of death and dying; paperwork preparation, collection of needed supplies; assessment of the patient's physical, mental, emotional and spiritual condition; documentation completed per policy; field phone calls from physicians, pharmacies and families; maintaining current clinical forms and patient information; call-rotation including weekday and weekend/holiday; complete PRN visits; collect supplies; keep current on policies, procedures, annual required updates, education, training and miscellaneous agency updates.

Successful candidates for the on-call shifts will arrive around 4 p.m. (weekend on-call perform phone call reports) for clinical update on all patients and new admissions; plus items listed below.

Position Summary: Responsibilities include paperwork preparation, collection of needed supplies; assessment of the patient's physical, mental, emotional and spiritual condition; documentation completed per policy; field phone calls from physicians, pharmacies and families; maintaining current clinical forms and patient information; receive assignments for the weekend/holiday; complete PRN assignments; collect supplies; keep current on policies, procedures, annual required updates, education, training and miscellaneous agency updates.

Education / Competencies

  • Valid, unrestricted North Carolina Nursing license.
  • Prefer two year’s experience; Hospice and/or Home Health experience a plus
  • Active patient contact within the past three years
  • Ability to assess and respond to the needs of patients and families in various settings
  • Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles
  • Ability to make meaningful observations and write comprehensive accurate reports;
  • Vehicle for Transportation
  • Local travel required with some regional travel for education/training
  • Knowledge of computer basics in Windows environment.

Minimal PRN Requirement:

RN's who work with our PRN Team set their own hours with a minimal requirement of one weekend per month.

Attend agency & departmental orientation and training; successfully pass medication competency

Work Environment

  • Office, health care facilities, and private homes.
  • Fast paced, multi-tasked position
  • Caseload varies day to day dependent upon needs of patients

Position Summary: Responsibilities include assisting patient with ADLs and home management tasks as outlined in the plan of care; paperwork preparation, collection of needed supplies; documentation; maintaining current clinical forms and patient information; keep current on policies, procedures, annual required updates, education, training and miscellaneous agency updates.

Education/Competencies

  • High School diploma or equivalent;
  • Completion and certifications of state approved training for home health aide (CNAI or CNAII)
  • Minimum one year experience in home care, LTC or hospital setting;
  • Ability to evaluate and respond to the needs of the patients and families in various settings;
  • Ability to make meaningful observations and document comprehensive accurate reports;
  • Ability to cope with family emotional stress and tolerant of individual lifestyles
  • Ability to make meaningful observations and report to appropriate employee in timely manner
  • Vehicle for Transportation
  • Local travel required with some regional travel for education/training
  • Knowledge of computer basics in Windows environment.

Minimal PRN Requirement:
