Family Resource Department

Family Resource Services Overview
The Family Resource Coordinators are available to work closely with both the patient and family. The role of the Family Resource Coordinator varies depending on the needs of the patient and the family. After determining the needs of the patient, the Family Resource Coordinator provides ongoing emotional support and supportive counseling which enables the family to prepare and adjust to the challenges and changes occurring in their lives.

Family Resource Services include:

  • Comprehensive psychosocial assessment to establish patient and family needs and concerns.
  • Home visits at least monthly with patients and their family.
  • Assistance with medical billing and insurance claims
  • Assistance with Family Medical Leave Act paperwork and Leave of Absence paperwork for family members
  • Advocate for the patient and patient rights
  • Family conflict resolution; regarding life choices and decisions
  • Referrals to community agencies for additional support
  • Financial assessment to link patient with available resources in times of financial hardships
  • Referrals to long term care facilities
  • Discharge planning
  • Support for the family during the bereavement period
  • Education and assistance with initiating and completing advance directives such as living will, and healthcare power of attorney.

Family Resource Coordinators are available to patients and their families on an as needed, individual basis. The Family Resource Coordinator will work with the patient and the family to alleviate problems in order for the family to provide quality care for the patient. It is our hope to assist families and provide support in times when they need it most.

  The Family Resource Department provides Bereavement Services.
